Accessibility Feedback Form

This feedback form is for reporting barriers you may have faced when accessing an LTSA service or facility. We also welcome your recommendations for accessibility improvements.

Please use this form to:

  • Describe your experiences interacting with LTSA employees or accessing an LTSA or subsidiary office.
  • Provide suggestions on how barriers could be removed or comment on LTSA accessibility features that are working well.
  • Give feedback on LTSA’s Accessibility Plan.
